What to Expect for your Enrollment Call
Thank you for your interest in Oregon Charter Academy! Our enrollment team will reach out via phone to go over the steps of the enrollment process and answer any questions you may have.
Please note: Although you are seeking enrollment at Oregon Charter Academy, you are encouraged to keep your child(ren) enrolled with their current educational provider until their enrollment with Oregon Charter Academy is finalized.
REQUIRED DOCUMENTS
An important step of the enrollment process is submitting required documents. Here are the documents you will need to submit to complete enrollment at Oregon Charter Academy:
Proof of Residency: Must be a picture of the full document. A mortgage statement, lease agreement with attached signature page, utility bill, or a letter from a government agency in the caretaker's name of the student enrolling.
All documents must be dated within 60 days, and if a utility bill is submitted, it must also include the following: SERVICE ADDRESS, PROPERTY ADDRESS, SERVICE TO, or BILL TO.
Proof of Age: Student’s birth certificate, passport, or a student’s driver’s license/State ID.
Please note: A social security card is not a valid document and is not required for enrollment.
District Notice of Intent to Enroll form: Please fill out the parent/guardian and student information and return to ORCA for processing. Requires an electronic or handwritten signature.
Oregon Certificate of Immunization Status: We must receive this form. Requires an electronic or handwritten signature.
Vaccination Education Certificate: If your child(ren) is not vaccinated for non-medical reasons, you will need to submit a Vaccine Education Certificate in addition to the Immunization Form in order to enroll in an Oregon public school. For further instructions and to obtain a certificate, please visit the Oregon Health Authority Vaccine Exemption website.
Most Recent Academic records:
Elementary & Middle students: Please submit their most recent report card.
High School students: Please submit a copy of their most recent unofficial transcripts that includes all credits earned in the previous school year and a progress report for their current courses.
Dental Screening & Vision Screening: Only required for students 7 years & younger. These forms do not need to be filled out by a doctor to be accepted.
